Case Management Nurse New

Pride Health is hiring a RN Case Manager to support our client’s medical facility based in Lexington, NC 27292. This is a 6-month assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!

Location: Lexington, NC 27292

Duration: 6 months Contract (Possibilities of extension)

Payrate: $43 - $45/hr. on w2/Pay Rate is based on experience and educational qualifications.

Schedule: Mon - Fri 8:00 am-5:00 pm (5 days/wk).

Job Responsibilities:

• Ensure accurate, complete documentation and coding.
• Communicate and collaborate with interdisciplinary care team
• Support care coordination activities and benefit determinations (Medicare A, B, D)
• Monitor and respond to changes in patient condition in collaboration with APC and facility
• Assist with therapy and medication coordination
• Attend facility-based meetings and family conferences
• Schedule and participate in team meetings and case presentations
• Support training, tracking, and quality measure compliance
• Conduct and assist with in-services for selected staff
• Assist with nursing rounds, assessments, and documentation
• Write provider verbal orders per state and facility regulations
• Manage patients during acute episodes or chronic condition exacerbations
• Support care during short-term sub-acute rehab episodes.

Requirements:

• Active and unrestricted license in the state which you reside, or ability to obtain by start date.
• Associate degree or higher in Nursing (RN)
• Certified in BLS.
• Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment.
• Access to reliable transportation.
